1. Who is responsible for your information

2. Information that may be collected
Depending on how you contact or book the service, information may include:
- your name, telephone number, email address and postcode;
- appointment preferences, visit address, parking and access details;
- booking, cancellation and payment information;
- messages and correspondence;
- relevant health, treatment, consent and aftercare records;
- limited technical security data needed to deliver and protect the website.
Please do not send detailed medical records through ordinary email. Relevant health information can be discussed through the agreed consultation process.
3. Why information is used

The online booking link opens a separate Sif Health booking service. Hosting, booking, payment, email, accounting, professional advisers, insurers or public authorities may process limited information where necessary. Each external service has its own privacy information and security responsibilities.

Information is kept only for as long as reasonably required for treatment continuity, professional and insurance requirements, tax and accounting obligations, complaints, legal claims and other lawful purposes. Enquiries that do not become appointments are deleted when no longer needed.
